This form should be filled out if the lab ONLY needs more containers for liquid chemical waste, solid waste, or infectious waste.
Please fill out the lab information (building and room number) on where to drop off the new containers.
Select from the drop down list which containers are needed. If more than one container is needed, please add additional containers.
DO NOT FILL OUT ANY CONTAINER CONTENTS! This section is not needed. Only select which new containers the lab needs.
