Non-Lab Materials Waste Pickup Request

Please fill out all required questions below to request a Non-lab Materials Waste Pick Up.

DO NOT FILL OUT ACCUMULATION START DATE or WASTE CODES.

In order to have the non-lab waste picked up, the following requirements must be met:

  3. Containers must be labeled correctly (i.e. Waste - Lead Paint).
  4. All waste must be in shippable/UN rated containers. If the container is not correct, a repack charge will be applied.
  5. If the requestor is not USC personnel, the Project Manager information must be filled out.
  6. If the fund code is known for the project, it must be filled out in its entirety.
  7. The "Container Type Description" should denote what kind of container it is (i.e. bucket, box, bag, etc.).
  8. The "Container Contents" should denote what item is being picked up (i.e. lead paint, batteries, aerosol cans, etc.).
  9. The "Percentage" should denote how many containers of that item you have (i.e. 5 buckets of lead paint, 15 aerosol cans, etc.).
