Please fill out all required questions below to request a Non-lab Materials Waste Pick Up.
DO NOT FILL OUT ACCUMULATION START DATE or WASTE CODES.
In order to have the non-lab waste picked up, the following requirements must be met:
- The outside of each container must be clean.
- Containers must be kept in a secure location.
- Containers must be labeled correctly (i.e. Waste - Lead Paint).
- All waste must be in shippable/UN rated containers. If the container is not correct, a repack charge will be applied.
- If the requestor is not USC personnel, the Project Manager information must be filled out.
- If the fund code is known for the project, it must be filled out in its entirety.
- The "Container Type Description" should denote what kind of container it is (i.e. bucket, box, bag, etc.).
- The "Container Contents" should denote what item is being picked up (i.e. lead paint, batteries, aerosol cans, etc.).
- The "Percentage" should denote how many containers of that item you have (i.e. 5 buckets of lead paint, 15 aerosol cans, etc.).
