Please fill out all required questions below to request a Peroxide Forming Chemical Waste Pick Up.
All standard hazardous waste requirements must be followed for Peroxide Forming Chemicals! This includes tagging as hazardous waste!!!
In addition to the standard hazardous waste requirements, the following must also be met:
- Please put the date of submission as the accumulation start date. This is the only time you will put an accumulation start date!
- Enter the full name of the peroxide former and list it as 100%.
- In the notes section, list any additional information for the chemical, such as ">100 ppm" or "crystals on lid."
- If you do not know the pH of the chemical, do not try to open the container to test it! Just leave the pH blank.
- Make sure you put at least one of the following dates: Last Date Opened, Last Date Tested, or Manufacturer Expiration Date.
USC EHS has the right to reject any hazardous waste containers that do not comply with the previous requirements listed above.
